Draw blood in a plain, red-top tube(s) or a serum-gel tube (s) from a fasting patient (preferred). Hemolyzed specimen is not acceptable. Spin down, separate from clot, and send 1mL of serum frozen in plastic vial.

CALCIUM, PHOSPHORUS and CREATININE

Plasma- draw blood in green-top (lithium heparin) tube from a fasting patient (preferred). Hemolyzed or lipemic specimen is not acceptable. Label specimen with patient name (first and last), date of birth, date and time of draw, phlebotomists initials.
