Specimen Requirements

Draw blood in a green-top tube(s) prior to morning dose. 

Note:  1. Include date, time, amount and route of last dose, and a list of other drugs patient is taking on request form.

2. Label specimen with patient’s name (first and last), date of birth, date and time of draw, and phlebotomist’s initials.

Store sample at 2-8C for up to 48 hours. Freeze sample for longer storage. Do not use sample that has been at room temperature for longer than eight (8) hours.
