Specimen Requirements

This test must be scheduled in advance with the Blood Bank. Call Augusta Campus at 207-621-3647 or Waterville Campus at 207-872-1854 to schedule.

Physician must note amount to be drawn, frequency of draw, and minimum hemoglobin and hematocrit level.

Note:  1. Record on request form pre- and post-phlebotomy vital signs.

2. Physician’s signed order is required for processing and will be kept on file in the Blood Bank.
