Submit only 1 of the following specimens:
Random Urine Collection
15 mL from a random urine collection in a plastic urine container. Send specimen refrigerated.
Note: Label specimen with patient’s name (first and last), date of birth, date and time of collection, collector’s initials, and as random urine.
Timed Urine Collection
15 mL from a timed urine collection. No preservative. Mix well before taking 15-mL aliquot. (An entire timed urine collection is also acceptable.) Refrigerate specimen during collection, and send specimen refrigerated in a plastic urine container.
Note: 1. Total timed volume is required on request form for processing.
2. Label specimen with patient’s name (first and last), date of birth, date and time of collection, collector’s initials, and as timed urine.
