Test Code URIN Urinalysis

Additional Codes

URINI- urinalysis, culture if indicated

URIN- urinalysis with microscopic if indicated

URINM- urinalysis with microscopic included

URINEC- Culture, Urine will be reflexed if nitrite is positive, leukocyte esterase is positive, WBCs are ≥5 WBC/HPF and/or  >1+ bacteria or yeast present. A urine will not reflex to culture if the squamous epithelial count is >11 per HPF.

Performing Laboratory

MaineGeneral Medical Center

Specimen Requirements

Submit 10 mL of urine from a “clean-catch,” random urine collection in a screw-capped, sterile container. Refrigerate specimen within 2 hours of collection, and send specimen refrigerated. Maintain sterility and forward promptly.

Note:  Label specimen with patient’s name (first and last), date of birth, date and time of collection, and collector’s initials.

Specimen rejection criteria:  Urine samples that are intensely colored with blood or medications which may cause inaccurate chemistry results will have only a microscopic performed. The chemistry dip will be cancelled and a new test reordered by the laboratory.

Day(s) Test Performed

Monday through Sunday

Test Classification and CPT Coding

81001 - with microscopy

81003 - without microscopy

Testing

Includes appearance, bilirubin, glucose, ketones, leukocyte esterase, nitrite, occult blood, pH, specific gravity, total protein, urobilinogen, microscopic examination, if indicated, and culture, if indicated.

Reference Values

Appearance:  yellow and clear

Bilirubin:  negative

Glucose:  negative

-Critical value (automatic call-back):  any amount detected (newborns:  0-14 days)

Ketones:  negative

-Critical value (automatic call-back):  any amount detected (newborns:  0-14 days)

Leukocyte esterase:  negative

Occult blood:  negative

pH:  4.8-7.5

Specific gravity:  1.003-1.030

Total protein:  negative

Urobilinogen:  0.2-1.0

Microscopic examination

Note:  The elements known as the "Big Five" will always be reported with a result whenever a microscopic is performed. If none are seen, the negative range will be reported.

Microscopic examination negative ranges:

RBC'S per HPF       0-2

WBC'S per HPF      0-5

Epithelial cells         0-2

Hyaline casts           0-2

Bacteria per HPF     none seen
