Specimen Requirements

Draw blood in a green-top tube(s). 

Trough level should be drawn just prior to next dose.

Note:  Label specimen with patient’s name (first and last), date of birth, date and time of draw, and phlebotomist’s initials.

Samples may be stored at 2-8C for up to two (2) days of freeze for longer storage.
